Description of services: CLIN 0001, Physicians, Period of Performance: 1 October 2008 30 September 2009, CLIN 0001AA, Cardiologist - 1920 Hours, CLIN 0001AB, Ophthalmologist - 2000 hours CLIN 0001AC, Anesthesiologist - 2000 Hours, CLIN 0001AD, OB/GYN Physician - 2080 hours, CLIN 0001AE, Radiologist 1073 Hours, Option CLIN 0001AF, Neurologist - 960 Hours, Option CLIN 0001AG Gastroenterologist - 960 Hours, CLIN 0002, Physicians, GWOT, Period of Performance: 1 October 2008 30 September 2009, CLIN 0002AA, Anesthesiologist (GWOT) 2000 Hours, Option CLIN 0002AB, OB/GYN Physician (GWOT) 960 Hours, Option CLIN 0002AC, Radiologist (GWOT) 960 hours, Option CLIN 0002AD, General Surgeon 960 hours, CLIN 0003 Physicians (TBI), Period of Performance: 1 October 2008 30 September 2009, Option CLIN 0003AA, Neurologist (TBI) - 960 Hours, CLIN 0004, Contract Manpower Reporting throughout the life of the contract. Performance incentives will be included for achieving predetermined performance targets. Option CLINs will be included for similar services and quantities that may extend contract performance through 30 September 2013. The initial period of performance is from 1 October 2008 through 30 September 2009. The formal request for proposals shall be issued shortly after this notice is posted and it is anticipated that offers will accepted through 29 August 2008.
