The contractor shall provide Physical Therapy Services to beneficiaries in accordance with the terms and conditions stated in the solicitation at the VA Nebraska Western Iowa Health Care System, (VA NWIHCS), Grand Island Division, NE, 2201 North Broadwell Avenue, Grand Island, NE 68803. It includes a base year with four (4) twelve-month option periods. The contractor will be required to sign-in/sign-out (including lunch time), identifying arrival and departure times. The contractor will be reimbursed for only those number of hours identified on the sign-in sheets. FTE is defined as a contractor's employee working a minimum of 80 hours every 2 weeks while physically present at NWIHCS. Under this contract, the estimated number of hours is a .5 FTE working the normal hours of Monday through Friday, 4 hours per day, preferred tour of duty is 8:00 am to Noon. The volumes or amounts shown in the solicitation are estimates only. The contract shall be for the actual requirements of the VA as ordered by the VA during the life of the contract. The minimum and maximum for the entire contract ordering period are: $3,000.00 (minimum) and $336,000 (maximum). QUALIFICATIONS: The contractor shall provide personnel based on the required qualifications: (1) Citizenship: Citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens. (2) Physical Requirements. See VA Directive and Handbook 5019. (3) English Language Proficiency. Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). (4) Licensure. Current, full, active, unrestricted license to practice physical therapy in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia. (5) Degree. A baccalaureate degree in physical therapy or an equivalent certificate in physical therapy from an accredited college or university and completion of a physical therapy clinical practice program. Both the education and clinical practice program must have been approved by the Commission on Accreditation in Physical Therapy Education and have been completed at an accredited college or university. Verification of approved programs may be obtained from the American Physical Therapy Association website. EXPERIENCE: Therapist should have experience to provide physical therapy services in a therapeutic setting such as: (1) Ability to evaluate and treat patients in the Nursing Home Care Unit and on an Outpatient basis. (2) Treat diverse diagnosis to achieve meaningful functional outcomes. (3) Interpretation of physicians' prescriptions. (4) Application and interpretation of standard evaluation procedures and techniques. (5) Development and implementation of physical therapy treatment programs for patients. (6) Working effectively with patients, families, friends, and treatment team members. (7) Use of standard and non-standard methods for evaluating and interpreting a wide variety of patient physical functional levels and providing progressive treatment. (8) Independently developing and implementing a physical therapy treatment program using judgment to modify treatment for changing medical conditions or because of a variety of severe physical disabilities. (9) Implementing treatment plans which directly affect the patient's social, economic, and functional well being. (10) Effectively communicating, both orally and in writing. (11) Working effectively with treatment team members and other health care providers in coordinating the patient's total care. DUTIES - GENERAL: The contractor employee will be an independent health care professional who is responsible for evaluating referred patients and establishing and executing treatment plans that include a full range of treatment procedures and modalities. These treatment plans are designed to restore individual patients to their optimal functional level and to enhance their adaptive capacities through the use of physical measures, activities, prostheses and/or assistive devices. The Physical Therapist must properly document patient records in a timely manner, in accordance with the contract provisions. The contractor shall be required to input all data into the Computerized Patient Record System (CPRS) in accordance with the following guidelines.
