Synopsis:

North Florida/South Georgia Veterans Health System (NFSGVHS), VA Medical Center, Gainesville, FL is seeking the award of a contract to furnish Perfusionist and Autotransfusion Services. The solicitation will be conducted in accordance with FAR Part 13.5 test program. The solicitation adheres to the format defined in FAR 12.303 for the acquisition of commercial products and shall be evaluated in accordance with the procedures of FAR Part 13.106. This solicitation is issued as a Request For Quotation VA-248-08-RQ-0348. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular FAC 2005-26. NAICS: 621399 with a size standard of $ 6.5 million. The contractor shall furnish all necessary supervision and qualified competent personnel to perform Perfusionist and Autotransfusion services in the treatment of care for eligible veteran beneficiaries as referred and authorized by North Florida/South Georgia Veterans Health System. Services will be performed within the VA policies and procedures of the NFSGVHS. All services provided under the contract shall be in compliance with the standards of The Joint Commission. The contractor shall provide services for the period October 1, 2008 - September 30, 2012 with 3 option year renewals. The Government intends to award a single award, firm fixed price contract for services in accordance with all terms, conditions, provisions, specifications, and schedules herein, pending the availability of funds. The Authority for this procurement extends from Public Law 104-262 and Title 38 U.S.C. Sections 8153. Services under the contract shall be performed on site at the VA Medical Center, Gainesville, Florida. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contract award may be based on initial offers received, without discussions. The solicitation will issue on or about July 15, 2008. The closing date for submission of proposals is August 18, 2008 at 3:00pm local time. Copies of the solicitation shall be acquired electronically. A limited number of hardcopies shall be provided if requested in writing and received by the issuing agency no later than 14 calendar days from the date this notice is published. No telephone or fax requests will be accepted. Hard copies shall be issued to the first 10 requestors. Any amendments issued to this solicitation will also be posted on FedBizOpps. Offerors are advised that they are responsible for obtaining amendments and acknowledging amendments.
