Description
This notice is not a solicitation announcement and will not constitute any commitment by the Government.     A market survey is being conducted to gain knowledge of potentially qualified small business sources to determine if they can effectively compet e for award of a Firm Fixed Price Contract with a base and four option years with an estimated value of $70,000.00 per year totaling $350,000.00. Only Small Business firms shall respond to this notice. This requirement covers Occupational Health Medical Se rvices for the U. S. Army Corps of Engineers in Wilmington (NC) and Charleston (SC) Districts.     Services will be provided to federal employees of participating agencies in accordance with the requirements. South Atlantic Division (SAD) Regional Occupationa l Health Center (ROHC) will provide direction for Occupational Health Medical services to the District Offices. The ROHC is located in the Federal Building, 109 St. Joseph Street, Mobile, AL.       Future Occupational Health Medical Services may be extended to     federal employees in the Districts of Jacksonville (FL), Savannah (GA, NC) and Mobile (AL, GA, TN, FL, MS). The Medical Surveillance Program was developed in accordance with the requirements of (OSHA), Department of ARMY (DA) and the Corps of Engineer wi th the purpose of protecting the health of the workers. Sources are being sought for firms with a North American Industry Classification System (NAICS) code of 621111.     The size standard for which is a concern, including its affiliates whose average annual     gross revenue does not exceed $9 million for the past three years.     Documentation and discussion should be submitted with the following considerations:     1. The Occupational Health Medical Services provider shall have been engaged in Occupational Medicine for a minimum of 5 years. Provide names and credentials of     a full time permanent, board certified Occupational Health Physician and a full-time, permanent, board certified (or board eligible) Occupational Health Nurse.     2.     Discuss capability of providing     full scale Occupational Health Clinic Services to multiple US Army Corps of Engineer locations throughout AL, FL, GA, MS, NC, VA, SC and TN.     Additionally, health promotions may be provided to our Puerto Rico office.     3.     The Programs for the Regional Occ upational Health Center have been developed in accordance with the requirements of the Occupational Safety and Health Administration (OSHA), the Department of Army, federal and state regulations along with the US Army Corps of Engineers.     The purpose of th e program is to protect the health of the worker.     Pre-placement examinations are used to ensure that potential employees are physically capable of performing the tasks required of them and to identify physical parameters for use in later comparisons.     Per iodic medical examinations are utilized to ensure that employees are able to perform his or her assigned task, and to provide early detection of any physical ailments that could progress to stages resulting in medical disability.     Additionally, the physica l examinations and evaluations are conducted prior to reassignment, and prior to retirement, resignations, transfer, or termination.     Discuss how the Contractor shall provide all personnel, supplies, and equipment necessary to provide all medical surveilla nce services required to District Offices.     4.     Contractor shall have the ability to review and evaluate all examinations and tests for completeness and accuracy prior to submitting to the US Army Corps of Engineers Regional Occupational Health Center (ROH C) - Provide discussion       5.     Provide discussion of how the following examination components would take place at any and all locations needed throughout our Southeast territory:     a.     Hands on Physical;     b.     Medical History;     c.     Vision (Near/Far);     d.     Aud iogram by CAOHC technician;     e.     EKG;       f.     CXR (PA & LAT);     g. B-reader for CXR;     h.     Blood collection (USACE contracts lab separately);     i. Blood Lime screeni ng where indicated ELISA test followed by Western blot analysis for positive ELISA.     j. PFT;     k. Td/Tdap as appropriate q10 years.     6. Discuss ability to complete DoD forms (i.e. SF78, respiratory questionnaires) on all physicals and provide a dual copy of     completed exam and results within 14 days.     Response can be submitted via email to Alisa C.
