Synopsis:

This project is a request for quotations to provide Mental Health Services at the Great Onyx Job Corps located in Mammoth Cave, Kentucky. All offerors must be registered in the Central Contractor Registration Database located at http://www.ccr.gov (FAR 52.204-7) and must have completed electronic representations and certifications on the ORCA database located at http://orca.bpn.gov (FAR 52.204-8) to be considered responsive to this solicitation. Mental Health Consultant shall be licensed by the state of West Virginia as a Psychiatrist, Clinical Psychologist, Psychiatric Social Worker, and/or Mental Health Counselor under the Supervision of one of the above. The project site for Mental Health Services is at the Great Onyx Job Corps located at 3115 Ollie Ridge Road, Mammoth Cave, Kentucky, 42259. The Government intends to award a firm fixed price contract for this requirement. The start of work date for the base year of services under this contract is July 1, 2008 and will run through June 30, 2009. This contract will have four option years that may be exercised at the option of the Government. A contract may be awarded with or without discussions resulting from the solicitation to the responsible offeror whose proposal conforms to the solicitation that will be most advantageous to the Government, price and other factors as outlined in the solicitation considered. This project is 100% set aside for small businesses. The NAICS code is 621112. The small business size standard is $9 million. All other requirements are included in the solicitation, which shall be posted on the FEDBIZOPPS website for downloading. The estimated price range for the base year is between $25,000 and $100,000. Paper copies of the solicitation will not be issued and telephone requests or fax requests for the solicitation will not be accepted. The solicitation will be available for download at no cost at www.fedbizopps.gov . We anticipate the solicitation to be issued on or about April 11, 2008 however this date may change and we invite all interested parties to check frequently for a possible change to this date. It is anticipated that award may be issued on or about May 30, 2008. All questions related to this solicitation must be submitted in writing to Christina Schofield at cschofield@fs.fed.us. Only written questions will be addressed. This amendment re-issues the solicitation and extends the offer due date to Thursday June 26, 2008.
