Synopsis:

The contractor is to provide Radiology and other Medical Imaging Services for the VAMC Lexington, KY for the Hazard VA Outpatient Clinic, 210 Black Gold, Blvd.,Suite 107, Hazard, KY 41701. The services are to be performed at the Contractor's facility which must be located within a thirty (30) mile radius of the Hazard VA Outpatient Clinic. The contract period will be for a base year and four (4) one-year options to renew. This is Total Small Business Set Aside commercial item procurement. The following commercial item clauses and provisions and any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4

Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders. NAICS: 621999. Solicitations will be issued o/a September 3, 2008.
