Department of Veterans Affairs (VA), Veterans Affairs Medical Center (VAMC)-Iowa City has a requirement to provide cryotherapy services on an as needed, per procedure basis. Contractor to provide cryosurgical apparatus and ultrasound scanner along with a certified ultrasound sonographer and certified cryosurgical equipment specialist trained and experienced in providing cryotherapy services for urological surgical procedures for the treatment of prostate cancer and for laparoscopic renal procedures for cryosurgical ablation of kidney tumors. The specialist(s) provided must be a certified Ultrasound Sonographer and a certified Cryoablation Systems Application Specialist. Contractor shall provide a certificate of training completion for each equipment specialist provided indicating that s/he is a trained and a certified sonographer and a cryo-applications specialist. All equipment and services shall be provided on site at the VA Medical Center, 601 Hwy 6 West, Iowa City, IA, on a per procedure basis. A VAMC physician will perform the actual surgical procedure. Contractor shall also provide the following: (1) Cryo-ablation system including all associated disposables (Gas hoses and regulators system (VA will provide gas), Urethra Warming Device, Doppler Ultrasound Scanner - Color, Bi-plane Ultrasound Transducer, Laparoscopic Ultrasound Probe, Linear Ultrasound Probe, Stepping & Stabilizing Device (floor or rail), 17 Gauge Grid Template) (2) Supplies to be provided per prostate procedure shall include: Disposable Cryo-Needles - 14 Each, Disposable Cryo Thermal Sensors - 3 Each, Disposable Urethra Warming Catheter & Tubing - 1 Each. (3) Supplies to be provided per renal laprascopic procedure shall include: Sharp-tip cryo-ablation probe, 180mm x 3mm - 1 Each, Sharp-tip laparoscopic cryo-probe, 400mm x 3mm - 2 Each, Renal Cryo-ablation Kit to include 17 Ga. Needles - 6 Each, Thermal Sensor Needles - 2 Each. Cryo-Ablation Professional Services required are Certified Cryo-System Technician and Certified Ultrasound Sonographer. The Contractor is responsible for keeping equipment in good working order and shall provide all maintenance, repairs and upgrades to the equipment as required maintaining it in optimal condition. Payment will be made on a per procedure basis. The base period will be from the date of award until the end of the current Fiscal Year (Sept. 30) plus two 12-month option periods. Total contract period not to exceed 36 months. Contractor shall be responsible for providing his/her own housing, transportation, malpractice insurance, professional membership fees, continuing education costs, sick leave and vacation time. All offers submitted will be referred to a Technical Evaluation Panel for review. Award of a contract will be based on the following: professional qualifications; clinical and consultation skills; availability; past experience and performance history; and total cost. Commercial Items terms, conditions, and provisions/clauses will apply. VA will conduct the procurement according to FAR Part 12, Acquisition of Commercial Items. Solicitation documents will be available on this website on or about March 27, 2008. This acquisition is subject to the availability of funds.
