Synopsis:

The Department of Veterans Affairs VISN 9 Contracting Office located at 3400 Lebanon Pike, Murfreesboro, TN 37129 is soliciting for the services of a company who is regularly established in the business of providing diagnostic medical testing and analysis services of sleep related respiratory disturbances using a portable, FDA approved/cleared, unattended home sleep testing and monitoring device, hereafter referred to as Home Sleep Testing device (HST).
Physicians from the James H. Quillen VAMC in Mountain Home, TN will will be the only VAMC referring veteran patients to the prospective awardee for diagnostic testing services under this solicitation. The Prospective Awardee will deliver and retrieve the Home Sleep Testing device (HST) to the referred veteran patient's residence, or other location(s) where the veteran patient may be temporarily located, including a hospital facility.

This requirement will be 100% set-aside for small business and is identified under NAICS Code 621498 with a small business size standard being $9.0 million. The solicitation will be issued o/a June 5, 2008 with offers due o/a June 18, 2008. All requests for the solicitation must be in writing by email (sherita.crosby@va.gov).
