The items listed hereinafter are hereby made a part of the
contract for the above mentioned project and shall govern the work taking
precedence over previously issued contract documents governing the items
mentioned. Receipt of this addendum is
to be acknowledged on page OF-8 of the proposer s packet.
1. Please replace the Statement of Work with the
following Revised Statement of Work.
2. Note:
To the best of the government s knowledge the conduit at the fuel
station goes to a junction box located next to the grease ramp located
approximately 100 southeast of the fuel station. Grease ramp is not depicted on construction
drawings. Conduit terminates at the
electrical panel LE4. Government cannot
determine at this time if work was completed.

The bid opening date has been changed to May 30, 2008 at 2:00PM.

STATEMENT
OF
WORK
INSTALLATION OF TWO (2) EACH 2000 GALLON
ABOVE GROUND FUEL TANKS FMS #1
Hawaii Army National Guard Facilities Kalaeloa,
Kapolei, Oahu, Hawaii

1. GENERAL.
a. TITLE:
Installation of Two (2) each 2000 gallon Above Ground Fuel Storage or one (1) 4000 gallon
Above Ground Fuel Storage split compartment tank with separate pumps and
controls, electric level probes for each compartment at FMS #1 Building 117 Hawaii Army National Guard
Facilities Kalaeloa, Kapolei, Oahu, Hawaii.
b. Scope of Work. The
work involves the purchasing of and installation of two (2) each above ground
fuel tanks, with pumps, meters and gauges or one (1)
4000 gallon Above Ground Fuel Storage split compartment tank with separate
pumps and controls, electric level probes for each compartment. This
is a turn key project upon completion AGST must be fully compliant and
operation in accordance with Federal, State, County codes and regulations.
Concrete pad which was part of another Military Construction Contract is in place
but may require installation of bollards and other peripherals to be compliant.
The new AGST will be 3/16 Steel Primary Tank,
Non-Metallic Insulated Secondary Containment, Manual 2 Leak detection tube, 6
reinforced 5000psi Concrete Protection, Tested for 2 hours fire, Ballistic,
Vehicle impact and Corrosion. Meets U.L. 142, U.L. 2085 Insulated secondary
Containment Protected Type Vented by Way of Construction, UFC 79-7, NEPA, 30 & 30A, CARB G-70-116. Monitor control
panel shall be in a NEMA 4 enclosure with audio and visual alarms min 90
decibles with a primary and secondary emergency shut off.
The new tank will have an epoxy based exterior
finish and be equipped with the following items: a seven gallon over spill
containment, 1 Guillotine fill limiter, set earthquake restraints and
anchor bolts, set NEPA signage for diesel, fuel dispenser with hose, nozzles
and trim, reel with 1 x 30 hose and ball stop, off tank bracket for
dispenser, primary and emergency venting, a monitor adapter and cap, a 2 test
well cap and adapter, a pressure/vacuum vent, inch gauge stick, a level gauge,
a decal package, a 2 x 8 vent pipe, a suction pipe, three (3) 2 couplers, a
2 cap, a 2 x 1 x 1 double tap bushing, a 3/4 anti-siphon valve and three
maintenance manuals.
The new AGST shall be able to pump 15 gallons of
fuel per minute.
c. Existing Drawings: Are not available.
d. Performance period: 180 days from notice to proceed.
2.
Investigation:
a. Accomplish the necessary field surveys
and investigations as required to consider the existing conditions. Inspection includes, but is not limited to,
determining the condition of the existing structure and components listed under
the scope of work. Note and locate all
items needing special attention. A site
visit will be scheduled by the government prior to bid.
b. Provide recommendations of any repairs
in addition to those specified that should be accomplished with the specified
work that is not included in this scope of work. The Contracting Officer shall negotiate any
additional costs prior to commencing any work.
Provide separate cost proposal of such work.
3. Specifications are
attached. Color selections shall be made
from standard manufacturer colors.
4. Bidding questions should
be addressed in writing to the Contracting Officer. Clarifications will be made in writing by
addendum by the Contracting Officer.
5. Close-out:
a. Provide Operations and Maintenance
manual information and shop drawings as needed for the described work, and new materials. Provide 3 each copies in 3 ring binders.
b. Final inspection to be scheduled with
the Project Manager at the conclusion of work after clean up. Contractor is responsible for prime and
subcontractor quality control.

9:30 AM on May 7, 2008 at Building 117, Kalaeloa. Please call Yale Alama at 672-1544 before 3:30
P.M. on May 6, 2008, to register for the walkthrough. It is strongly recommended, but not required, for all interested
bidders and subcontractors to attend

will be received in the Engineering Office, Department of Defense, State of
Hawaii, located in Building 306-A, Room 228, 3949 Diamond Head Road, Honolulu,
Hawaii, up to 2:00 P.M. on MAY 6, 2008
and will then and there be publicly opened and read aloud. Proposals may also be mailed to Department of
Defense, State of Hawaii,

for opening will not be considered.
Proposed
work consists of, but not limited to the following: Furnishing and installing two (2) new 2000 gallon above ground fuel
storage tanks (AGST) with pumps, meters and gauges. Upon completion the AGST
must be fully compliant and operational in accordance with Federal, State and
County codes and regulations.
The
estimated cost is between $100,000
and $160,000.
A
notice of Intent to Bid must be filed at least ten (10) days prior to the bid
opening for a bid to be considered. The
Intent to Bid may be mailed or hand carried to the above address or faxed to
733-4235.
All
requests for clarification of bidding documents and/or specifications must be
received in the office listed above, in writing, 14 days prior to bid
opening. Questions may be faxed to

Bona
