The Federal Bureau of Prisons (BOP) is soliciting quotes from sources possessing a current certification to provide OUT-PATIENT substance abuse treatment in the vicinity of the Residential
Re-Entry Center located at 300 Wendell Court, Atlanta ,GA. 30336.
Location should be at a reasonable walking distance and accessible to a local public transportation route for female and male Federal offenders held under the authority of various United Statutes under Request for Quote (RFQ) TDAT-012-8. The services are to be provided in accordance with the enclosed Statement of Work entitled, Federal Bureau of Prisons Community Transitional Drug Abuse Treatment (TDAT) dated May 2007".

The following MUST be completed and submitted with your offer:

Standard Form (SF) 1449- Solicitation/Contract/Order for Commercial Items - Please complete blocks 17a, 23, 24, 30a, 30b, and 30c of SF-1449 and the continuation sheets. Please include your telephone number in block 17a. Original signatures are required on quotes submitted.

Representations and Certifications (FAR 52.212-3) - Prospective offerors are required to submit Annual Representations and Certifications via the Online Representations and Certifications Application (ORCA) website at http://orca.bpn.gov. By submission of an offer under this solicitation, the offeror acknowledges completion of the annual representations and certifications via ORCA and verifies, by submission of the offer, that the representations and certifications currently posted in ORCA were entered or updated within the last 12 months and are current, accurate, complete and applicable to this solicitation. All offerors MUST be capable of and comply with FAR 52.204-7, Central Contractor Registration (Oct 2003) and FAR 52.2044-6 Data Universal Numbering System (DUNS) Number (Oct 2003). Submission of your offer constitutes compliance.
