This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number to be used is VA-250-08-RQ-0258 and is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation will result in a firm fixed price commercial supplies purchase order under the NAICS Code 339112. The solicitation document and the incorporated provisions and clauses herein are those in effect through Federal Acquisition Circular 2005-26.
The Department of Veterans Affairs (VA) Dayton Medical Center is looking for a vendor to provide the following items:
1) One (1) Liebel-Flarsheim Optivantage DH suspension system kit (Tyco Healthcare/Mallinckrodt Part Number V8402).
2) Optibolus Shaping Software (Tyco Healthcare/Mallinckodt Part Number 844200).
Please provide quote for the requested items or the equivalent to the specifications of the requested items. Include in your quote the following information: installation (if applicable); training (if applicable); warranty (parts and labor), estimated shipping and handling, Delivery (ARO), and availability of service and operator's manual(s). Include GSA Contract # (if applicable), DUNS #, Tax ID number and fax number.
Preference will be given to SDVOSB and VOSB.
VA will base their award on the lowest price, most technically acceptable offer. Award will be made on an all or none basis; whereas the awardee must be capable of providing all line items to be eligible for award.
Delivery will take place on or before the 30th day after the confirmation of the award, and the address will be the Dayton VA Medical Center Warehouse, Building 126 at 4100 West Third Street, Dayton, OH, 45428.
The provisions at FAR 52.212-1, Instructions to Offerors - Commercial, FAR 52.212-4, Contract Terms and Conditions - Commercial Items, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition.
