Description
The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Bronx New York, Network Acquisitions and Logistics program is currently seeking firms with the ability to provide Milk & Related products to the VA Medical Centers listed. ORDERING/ DELIVERY FACILITIES FOR VETERANS AFFAIRS HEALTH CARE SYSTEM 130 WEST KINGSBRIDGE ROAD BRONX, NY 104688 / CASTLE POINT CAMPUS NUTRITION AND FOOD SERVICES (120) BLDG. # 16 LOADING DOCK CASTLE POINT, NY 12513 / VA HUDSON VALLEY HEALTH CARE SYSTEM MONTROSE (FDR) CAMPUS BACK DOCK OF BLDG. # 5 MONTROSE, NY 10548 / DEPT. OF VETERANS AFFAIRS MEDICAL CENTER 79 MIDDLEVILLE ROAD BLDG. 200 MAIN KITCHENS NORTHPORT, NY 11768. Products listed in Milk & Related Products must meet the specifications. Applicants must have at least FIVE YEAR OF SPECIALIZED EXPERIENCE, Preferred if the applicant had provided same or similar products to other VA Medical Center throughout the United States. The applicant must also provide "REFERENCES" for providing same or similar products. The firm will be responsible for providing all management, supervision, labor, materials, supplies and equipment, and shall plan, schedule, coordinate and assure effective delivery of these products. Firms desiring consideration shall email directly to the Contracting Officer referencing the following in the subject field, "Milk & Related Products", then contain the following information within the body of the e-mail name of firm, complete address, phone & fax number of contact.
