Notice Summary
The Oregon Military Department, hereinafter referred to as the Agency, is requesting
proposals from qualified contractors to provide medical services to the students of the
Youth ChalleNGe Program located in Bend, Oregon. The awarded contract shall be for a
period of 1 year and may be extended for an additional one year period, for a total
contract period of up to 2 years.

The Youth ChalleNGe Program (YCP) motivates at risk youth by providing training to
complete educational credit toward high school, providing a GED exam preparation course,
and providing job skill training. Participants in the program may be either male or
female, must be between 16-18 years old, and must have dropped out of school within the
last 24 months. The student must be 1) an Oregon resident, legal resident of the United
States, or both, 2) free of drugs, and 3) not currently in the court system.

The curriculum combines classroom work, community service, and challenging individual and
team activities. Instruction includes health and drug abuse awareness, leadership and
self-discipline, personal development, physical fitness, and basic work skills. The
objective of the program is to graduate or reintegrate the students back into the
mainstream education system with the skills to successfully define their place in the
adult social structure. The program consists of an initial 22 week resident phase at the
YCP facility in Bend, Oregon. The second phase of the program is a 12 month nonresident
program. Program continuity is provided by incorporating advisors from the community as
mentors to students in their continuing efforts to meet educational and/or career goals
set during the residential phase.

Minimum qualification requires a valid Nurse Practitioner license with certification in
family practice, trauma care and internal medicine, or valid physician's certification and
current CPR or First Aid certificate.

The contractor shall provide medical services to participants (students) in the Youth
ChalleNGe Program (YCP) at the YCP facility located at 23861 Dodds Road, Bend, Oregon
97701. Contractor shall provide on-site medical services five days per week for a total
of 20 hours weekly. Contractor's service hours shall be Monday through Friday during the
daytime and shall be scheduled to not conflict with classroom curriculum.

Contractor shall provide scheduled on-site sick call visits to program students for acute
and chronic illness evaluations on three of the scheduled five days per week, and provide
facilitated referrals to outside health care providers as needed. Contractor shall
provide unscheduled on-site visits to treat emergency sick calls and non-life threatening
injuries. The contractor shall also provide on call emergency visitations and YCP staff
advice/consultation referrals 24 hours per day, 7 days per week with access by telephone,
cellular phone and pager. Contractor shall return all calls and pages within 30 minutes.
Contractor shall maintain cumulative health files and computer records for all students.
