Opportunity Summary
INTENT TO SOLE SOURCE JUSTIFICATION

Per OAR 125-247-0275; The Oregon Employment Department (OED) intends to enter into a Sole
Source contract with Inquisite, Inc. to provide web based software program software for
the purpose of conducting surveys. This survey software is used for surveys of staff,
clients and customers, and other entities outside of the Department.

OED intends to purchase a minimum of 9 licenses from Inquisite, Inc. ranging from
Administrative licenses to user licenses. These licenses will be shared between OED and
the Department of Human Services users.

Inquisite, Inc.'s survey software is found to be a sole source under 125-247-0275 (2)(A)
Compatibility for the following reasons:

OED has been using the Inquisite survey software since 1999 and the licenses to be
purchased will be in addition to what is currently in place and will upgrade the version
of the software OED currently has in place. No other manufacturer's software will
interface with Inquisite's software and no other vendor is capable of maintaining and
upgrading Inquisites software licenses. The purchase shall include a minimum of one year
of maintenance.

There is an ongoing need for this software and the maintenance of it. It is expected that
the licenses and maintenance for the current need shall be approximately $31,000 with
ongoing licenses and years of maintenance added as needed through future years.

Firms that believe they can provide this software and the associated maintenance may
submit a written request to Shannon Rand, Employment Department Procurement Manager, 875
Union Street NE, Room 310, Salem, OR 97311 prior to 2:00 pm Pacific Time on the closing
date and time of this notice, that clearly states why these services should be
competitively solicited.
