NOTICE TO PROPOSERS FOR REQUEST FOR QUALIFICATIONS
AND REQUEST FOR PROPOSALS

The
Department of Accounting and General Services (DAGS), State of Hawai i, on
behalf of the State of Hawai i, Department of Defense and the Hawai i Army
National Guard (HIARNG), hereby solicits, via a 2-step procurement process, qualification
proposals (Step 1) and proposals (Step 2) from Design-Builders for the design
and construction of the Keaukaha Military Reservation, Joint Military Center,
Phase I.
A compact
disk which contains the Step 1, Request for Qualification package may be
obtained from the Public Works Division Office, fourth floor of the Kalanimoku
Building, Room 422, 1151 Punchbowl Street, Honolulu, Hawai i or at the Hawai i
District Office, Department of Accounting and General Services, State Office
Building Basement, 75 Aupuni Street, Hilo, Hawai i. The Request for Qualifications may also be
available in electronic form from the Department s website

A
voluntary Pre-qualification conference was held at 9:00 a.m., on Monday,
October 29, 2007 at the Kalanimoku Building, Room 322B/322C, Honolulu, Hawai i,
to answer questions regarding the Request for Qualifications. All interested Design-Builders were advised
to attend the pre-qualification conference.
Sealed
qualification proposals were received at the Department of Accounting and
General Services, Public Works Division Office, fourth floor of the Kalanimoku
Building, Room 426, 1151 Punchbowl Street, Honolulu, Hawai i 96813 up to 2:00
P.M. Hawai i Standard Time, November 16,
2007. Sealed qualification proposals
received after the due time and date were not considered. Submission by Facsimile were not accepted.
DAGS notified
all pre-qualified short-listed Design-Builders and requested that each submit a
Notice of Intent to Submit Proposals and a Security Deposit/Bond in the amount
of $50,000.00. The Security Deposit/Bond
secured the Design-Builder s obligation to submit a proposal in accordance with
the terms of the Request for Proposals (RFP).
The
following Design-Builders have submitted the required Step 1 pre-qualification
information and have been determined to be qualified to proceed on to Step 2
and submit a proposal in accordance with the terms of the RFP. By this addendum, the DAGS acknowledges that
the below listed qualified Design-Builders have filed a written notice of
intention to submit a proposal, and have submitted a $50,000.00 security
deposit/bond satisfactory to the DAGS. Only these Design-Builders may submit
proposals for consideration by the DAGS.

Design-Builders
shall attend a mandatory pre-proposal conference. The pre-proposal conference will be held at
the United States Army Reserve Center, Fort Shafter Flats, Building 1554, First
Floor Assembly Hall, Honolulu, Hawai i, on February 20, 2008 at 9:00 A.M. A map showing the location of the conference
site is attached at the end of this notice.
Sealed RFP
proposals from qualified Design-Builders will be received at the Department of Accounting and General Services, Public
Works Division Office, fourth floor of the Kalanimoku Building, Room 426, 1151
Punchbowl Street, Honolulu, Hawai i 96813 up to 2:00 P.M. on May 2,
2008 . Sealed RFP proposals received after the due
date and time will not be considered. Submission by Facsimile will not be accepted.
The Design-Builder is responsible to
ensure that their RFP proposal transmittal letter is time stamped by the DAGS
personnel, upon delivery of the proposal documents to the Department of
Accounting and General Services, Public Works Division, Room 426.
The project
generally consists of the design and construction of the following facilities
of approximately 105,600 square feet of buildings, demolition of existing
structures, and related site work improvements on an 15.5 acre site within the
Keaukaha Military Reservation, that will service the Army National Guard
(HIARNG), United States Army Reserve (USAR), and Hawai i Office of Veterans
Services (HOVS).
1.
An Armed Forces Reserve Center,
including an assembly hall, classrooms, learning centers, training device/simulation
center, recruiting office, locker rooms and showers, kitchen and break room.
2.
Hawai i Office of Veterans Services office space.
3.
State Maintenance and HIARNG Environmental office and
work space.
4.
Maintenance workbays, tool, supply and battery rooms and
offices.
5.
Privately owned vehicle and military vehicle parking.
6.
The necessary site and utility work, and the
demolition of existing buildings being displaced by the new construction.
The
project site which is currently occupied/used by the Hawai i Army National
Guard is located near the Hilo Airport and is
identified by Tax Map Key 2-1-12: 03.
The
project control budget is $51,268,000.00. Federal Base Realignment and Closure
(BRAC) funds and State of Hawai i Capital
Improvement Project funds have been appropriated for this project.
The
project is subject to Hawai i Product Preference (HAR 3-124
Subchapter 1). The Hawaii Product List
may be examined at the State Procurement Office or online at
www.hawaii.gov/spo/SPO/Code/index_html.
To be
eligible to submit sealed qualification proposals, the Proposer must possess a
valid State of Hawai i
Contractor s license classification B .
Tax
clearance and Hawai i Business Certificates
As a
prequalification condition and condition of award for proposals of $25,000 or
more, Proposers shall have valid tax clearance certificates from the State
Department of Taxation and the Internal Revenue Service or shall have Special
Letters , that comply with the Bidding
and Execution of Contract Requirements Tax
Clearance section or be registered and compliant with Hawai i Compliance
Express, http://vendors.ehawaii.gov/hce/splash/welcome.html. Include the tax clearance certificates or
Special Letters prior to award.
As a
prequalification condition and condition of award for proposals of $25,000 or
more, Proposers shall be incorporated or organized as a Hawai i business or a
compliant non-Hawai i Business and shall submit prior to award, certificates
from the Department of Taxation (DOTAX), Department of Labor and Industrial
Relations (DLIR) and Department of Commerce and Consumer Affairs (DCCA),
Business Registration Division (BREG) or be registered and compliant with
Hawai i Compliance Express, link found at

REQUIREMENT
FOR CONTRACTORS LICENSING CLASSIFICATIONS
Design-Builders
holding an A or a B license are reminded that due to the Hawai i Supreme
Court s January 28, 2002 decision in Okada Trucking Co., Ltd. v. Board of
Water Supply, et al., 97 Haw. 450 (2002), they are prohibited from
undertaking any work, solely or as part of a larger project, which would
require a Design-Builder to act as a specialty Contractor in any area in which
the Design-Builder has no license.
Proposers are solely responsible to review the Request for
Qualifications, Request for Proposals and project requirements, determine the
appropriate licenses required, and ensure that they possess and that their
Subcontractor(s) listed in their SOLICITATION, OFFER AND PROPOSAL FORM possess
the necessary specialty licenses to perform the work for this project.

CAMPAIGN
CONTRIBUTIONS BY STATE AND COUNTY CONTRACTORS
Proposers are hereby notified of the applicability of
Section 11-205.5, Hawai i Revised Statutes (HRS), which
states that campaign contributions are prohibited from specified State or
county government contractors during the term of the contract if the
contractors are paid with funds appropriated by a legislative body.
Any protest shall be submitted to the Comptroller. Proposers shall comply with the GENERAL
CONDITIONS Article 2.12 Protest.
