This announcement constitutes a Source Sought Synopsis for the Navajo Area Indian Service. The information provided is to assist the Indian Health Service in conducting Market Research of Industry to identify potential contractors for this effort.

The applicable NAICS Code is 721214, which applies to industry primarily engaged in Recreational Camps.

The contractor shall provide all management, supervision, labor, material, supplies, equipment, transportation and plan, schedule, coordinate and assure effective performance of the operation of Recreational Camps for Native American School-age children prone to obesity/overweight and potential candidate to diebetes, and/or local organization participating in the Running & Fitness Camp programs for Indian Health Service, Northern Navajo Medical Center, Shiprock, New Mexico. The Key elements approach for the camp is to provide fun, safe, drug free, and educational environment for children to experience the joy of Physicial Activity. The camp will target children ages 8 to 18. Facilitators will serve not oly as role models, but to proivde a social network and social support to health for the children. Core components of the camps should include education on diabetes prevention, substance abuse prevention, physicial acitivity, nutrition, and the integration of Navajo traditional culture to these components. The recreational camps to be located within 100 miles radius of the boundaries Navajo Nation.

The contractor shall coordinator all activities with the NAIHS Health Promotion Program located at Northern Navajo Medical Center. Shiprock, NM.

The Government does not intend to make an award on the basis of this Source Sought Synopsis or otherwise pay for the information solicited herein.

This synopsis is for the purpose of identifying potential sources and does not commit the government to make an award and/or issue a solicitation.

Market Survey Questions:

Answer the following capability questions related to how your company has the necessary technical skills, expertise, and knowledge to successfully propose and, if awarded a contract, to perform this type of work.

1. Address and Point of Contact: What is the physical address of your main corporate office and who is primary point of contact?

2. Geographic Coverage: Please identify the areas of the United States where your organization operates.

3. Membership/Registration:

a. Is your company registered in the Central Contractor Registration? All contractors who wish to do business with NAIHS must be registered with CCR. You can register at the following website: http://www.ccr.gov/.

b. Is your company registered with the SBA as either 8(a) or HUB Zone business?

5. Does your company provide: Recreational Camp for Youth and/or local organization participating in the Running & Fitness Camp programs.

6. References: Provide a list of all private industry or government contracts for similar services that you have performed during calendars years 2006 through 2007. Please include the customer's names, addresses, telephone number, dollar value of contract, and brief description of the services provided on the contract.
