Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemental with additional information included in this notice. This announcement constitutes the only solicitation and a quote is being requested and a written solicitation will not be issued. This will be a firm fixed priced contract. This is a 100% small business set-aside.

The contractor shall provide all personnel, equipment, tools, materials and transportation necessary to perform monthly maintenance and unscheduled remedial maintenance on equipment on equipment identified in this Statement of Work (SOW).

The period of performance will commence the date the order is signed and continue for a period of one year.
All parts needed for repair and replacement parts that are no longer under warranty will be purchased by the Fitness Center but must first be approved by the Fitness and Sports Director or the Assistant Fitness Director.
The contractor will respond within 24hrs after being notified by authorized personnel. All repairs will be performed during normal duty hours, Monday through Friday between 0700 to 2200 hours, 0900 to 1600 hours on weekends and scheduled holidays. Any deviation from these schedules will be coordinated with the Fitness and Sports Director or his designated representative.
Parts required to maintain or fix equipment should be ordered No Later Than (NLT) 24 hours after service call. Parts should be received from the manufacturer by the most expeditious means possible; normally NLT 7 to 10 days after order is placed. Installation should be done NLT the next day after receipt of parts.
All bikes, benches, lifefitness, bodymasters, hammer strength equipment and weight benches that require fabric covering will be accomplished within a 24 hour period and returned the next duty day.
Lifefitness and Flex Equipment: Clean inside protective covers, clean chains and coat with Teflon protective coating. Check nuts and bolts for looseness; lubricate guide rods with light oil and check chains for tightness.
Bodymaster Equipment: Oil moving parts, check belts for wear, clean and oil guide rods.
Cardiovascular Equipment: Lubricate chains, tighten bolts and fly wheels, check computer for accuracy.
Elliptical Trainers: Lubricate slides and chains.
Remove and replace all deteriorated handgrips.
Reupholster any pads, cushions, seats, and benches that are worn or which are showing to have excessive wear.
Remove unserviceable, non-slip pads and replace.
Ensure all alien head screws are securely fastened on all 25 pair of dumbbells.
On a semi-annual basis the following equipment must be sand blasted or equivalent operation to remove rust and prepare for painting. Machines or equipment will be painted in epoxy white, black, or grey, which matches manufacturers original color scheme.

Preventative maintenance shall be performed to maintain equipment according to manufacture specifications/recommendations. The following list identifies equipment covered under this SOW and requirements for preventative monthly maintenance.

Labor to be covered under the Monthly Maintenance Charge.
