| In accordance with Chapter 60A-1.045, Florida Administrative Code, the Florida Department of Health, Division of Disability Determination requests authorization to procure telephone and voice mail maintenance for our Tallahassee and Jacksonville office locations. The phone sytem we use is an Iwatsu system which can only be maintained by a authorized Iwatsu distributor.
