The Department of Veterans Affairs Medical Center (VAMC), Hampton, Virginia is seeking potential sources capable of providing Homeless Shelter Services to include: Psychiatric Rehabilitation, Social Work and Community Based Health Care Services to four (4) beneficiaries for whom such care is specifically authorized by the VAMC. The Contractor shall provide all services to include: Structured group activities, including physical activities as appropriate. (2) Instruction in and assistance with health and personal hygiene (3) Monitoring of medications, including locked drawers for storage of medication. (4) Supportive social services, in collaboration with the Health Care for Homeless Veterans (HCHV) case managers, or other contract resources. (5) Individual professional counseling, including counseling on self care skills, adaptive coping skills, and as appropriate, vocational rehabilitation counseling, in collaboration with the VA's Compensation Work Therapy program, Supportive Employment Program or Incentive Therapy programs. (6) Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. (7) Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. (8) Assistance in learning, testing and internalizing knowledge of the illness/recovery process for homelessness. (9) Assistance with application for Social Security Disability or other income source for disabled veterans deemed incapable of gainful employment. (10) Assist with arranging transportation to care at the VA Medical Center and other appointments. Contractors' facility must be located within 50 miles of the VAMC. Contractor must provide as a minimum an administrative staff member or designee of equivalent professional capability on duty on the premises or residing at the facility and available for emergencies 24 hours a day 7 days a week. The staff must have current certification in First Aid and CPR; a Certified Nursing Assistant (CAN) or individual with medication training/certification. The NAICS code for this requirement is 624221. Size standard is $6.5 Million. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the socioeconomic categories and any pertinent information which demonstrates firms ability to meet the above requirement.
