| Description: |
The Massachusetts Department of Public Health (MDPH) through the Bureau of Community Health Access and Promotion is applying to the Centers for Disease Control and Prevention for a State Cooperative Agreement for Building a Healthy Nation-Strategic Alliance for Health (CDC-PA-DP08-813). The Bureau intends to contract with Neustras Raices in Holyoke and Partners for a Healthier Community in Springfield as the state-coordinated small city applicants who will create healthier communities through sustainable, innovative, evidence and practice-based community health promotion and chronic disease prevention programs that promote policy, systems and environmental change.
CDC's Healthy Nation-Strategic Alliance for Health serves to:
1. Promote physical activity and nutrition;
2. Reduce tobacco use and exposure;
3. Build capacity for communities to be able to institute systems, environmental, organizational and policy changes related to these health risk factors;
4. Foster improved and increased access to quality care;
5. Help eliminate racial and ethnic health disparities; and
6. Reduce complications from and incidence of cardiovascular disease, diabetes, and obesity.
Requirements for state-coordinated small city and rural community applicants:
If funded, in the first year of the project, the Massachusetts Department of Public Health (MDPH) will begin a community assessment of Holyoke and Springfield.
Program Requirements:
If funded, in years two through five, both Neustras Raices and Partners for a Healthier Community will conduct the following activities:
> Build an alliance of partnerships and coalitions committed to participating actively in the planning, implementation, and evaluation of the Strategic Alliance for Health Communities.
> Provide and collect data on existing policy, systems, organizational and environmental change strategies related to chronic diseases in the intervention area.
> Develop leadership, coordination, and management teams.
> Implement community-based intervention strategies.
> Implement School-Based Intervention Strategies.
> Develop a Community Action Plan.
> Enhance Capacity for Evaluation to Monitor/Measure Progress.
>Work in Collaboration with Other Chronic Disease Programs
Mentoring Requirements:
If funded by CDC, Neustras Raices and Partners for a Healthier Community will also serve as mentors to at least two other communities each per year during years 3 to 4 around effective policy, systems, organizational and environmental changes that will combat chronic diseases.
Cost Sharing Requirements:
If funded by the CDC, Neustras Raices and Partners for a Healthier Community will participate in required cost sharing to ensure future sustainability. Each organization will match 30% of its awarded funds in year two, increasing to 45% by year five.
This Notice serves two purposes: to publicize the MDPH's intent to contract with Neustras Raices and Partners for a Healthier Community and to invite any other organization that can demonstrate that it already meets the criteria noted above and has the ability to carry out all of the activities described to submit a proposal. Proposals must include scope of services, deliverables, budget costs for carrying out these activities, and documentation of capacity and experience to carry out all of the activities. Proposals must be submitted no later than June 19, 2008 at 4:00 pm.
