The following form must be completed. All fields are required unless otherwise specified.

Any addenda issued, pertaining to solicitations that have been downloaded, will be forwarded in the form of an e-mail to the e-mail address supplied on this form.

For construction related projects, Contractors will be sent, via email, a notification only that an addendum has been issued. A copy of the addendum will be sent to all Plan Holders via US Mail.

Bidders are reminded to frequently monitor their e-mail prior to the solicitation opening date. It remains the bidders responsibility to assure all addenda are acknowledged within their bid submittal.
