SCOPE OF WORK:
Work shall be performed at the Guthrie Job Corps Center; a Federally funded vocational/educational training facility operating year around. The Center Dentist is an independent contractor, and not an employee, representative, or agent of the Guthrie Job Corps Center.
The Center Dentist will provided services not to exceed thirty-two (32) hours per week, during a schedule determined by the Center Dentist and the Manager of the Wellness Center to meet the health requirements of the center enrollees.
QUALIFICATIONS:
Certifications, licensing or accreditation as required by the state of Oklahoma.
Experience in providing dental care for adolescents and young adults.
Provisions of dental services to youths who are economically and socially disadvantaged.
Previous practice in residential settings such as schools, colleges, institutions, military and correctional institutions.
Willingness and ability to carry out duties and responsibilities equal to those specified in the attached sub-contract.
