NOTICE TO PROVIDERS
OF PROFESSIONAL SERVICES
In
accordance with Section 103D-304, HRS, the Department of Transportation (DOT),
seeks a professional and qualified environmental engineering firm to evaluate
and audit the DOT s Environmental Management System
(EMS).
The general
scope of work will include the preparation of an EMS Audit Plan; conduct an EMS
Audit for Highways, Harbors and Airports (to include facilities on the neighbor
islands); prepare an Audit Report; prepare a Corrective Action Plan; and provide
EMS audit training for up to 12 personnel.
These services are
anticipated to have a contract completion time of one (1) calendar year from
the notice to proceed.
Individuals or
firms on the DOT s qualified list shall submit a
summary of their qualifications, which shall include the following:
1.
Experience and professional qualifications of key personnel who will be
assigned to this project, including without limitation, the project manager and
subconsultants;
2.
Past performance on projects of similar scope for public agencies or
private industry, including corrective actions and other responses to notices
of deficiencies;
3.
Capacity to accomplish the work in the required time; and
4. Ability to
satisfy the qualification requirements as outlined in Paragraph 18 and 19 of Appendix
E of the Consent Decree entered on January 30, 2006 (Civil Action no. CV0500636
HG KSC) between the State of Hawaii Department of Transportation (HDOT), State
of Hawaii Department of Health (DOH) and Environmental Protection Agency (EPA).
Individuals or firms who
are not on the DOT s qualified list, but who wish to
be considered in the selection for this project must submit, along with the
summary of qualifications as described above, four (4) copies of the
following:
1.
Letter of interest (including an email address
for notification of additional advertisements.)
2.
DPW Form 120 or the Standard Federal Form
330. The DPW Form 120 may be downloaded

Professional Services .
3.
Any other related information.

The DOT will use the following
evaluation criteria to evaluate firms who submit the required information by
the above deadline:
1. Experience and professional qualifications
relevant to the project - 35%
2. Past performance
on projects of similar scope for public agencies or private industry, including
corrective actions and other responses to notices of deficiencies - 33%
3. Capacity to accomplish the work in the
required time - 32%
The Department of Transportation, in accordance with the provisions of
Title VI of the Civil Rights Act of 1964 (78 Stat. 252) and the Regulations of
the U.S. Department of Transportation (Title 49, Code of Federal Regulations
Part 21) issued pursuant to such Act, hereby notifies all firms that it will
affirmatively ensure that the contract entered into pursuant to this
