NOTICE TO PROVIDERS OF PROFESSIONAL
SERVICES
In
accordance with Section 103D-304, HRS, the Department of Transportation (DOT),
Airports Division seeks the services of qualified construction management firms
to provide construction management services for the Strengthen 2nd
Level DH & Ewa Turnarounds project at Honolulu International
Airport, Oahu,
State Project No. AO1013-14. The scope of work for this project consists
of the installation of additional steel beams beneath the double-tee and
tri-tee flanges of the existing structure and other related work. These
services are anticipated to be required by January 2009, and will have a
contract completion time of approximately two hundred seventy (270) calendar
days from the notice to proceed. The
estimated construction cost is $1,000,000+.
Individuals or firms interested in being
considered as the Prime CM Consultant for this project and who have the
necessary experience and qualifications must respond to the written
questionnaire which can be viewed by clicking More Info and then on View Specifications
Document. Only those prospective individuals or firms who submit their response to the
written questionnaire by the deadline will be considered. The response to the questionnaire will be
evaluated by the following criteria:
1. Experience
and professional qualifications relevant to the project - 35%
2. Past performance on projects of similar
scope for public agencies or private industry, including corrective actions and
other responses to notices of deficiencies - 33%
3. Capacity to accomplish the work in the
required time - 32%
Subconsultant
submittals are not necessary and will not be factored into the evaluation. Individuals or firms who have not submitted
their DPW 120 or Standard Federal Form 330 for Fiscal Year 2008, and wish to be
considered in the selection for this project must submit four copies of the
following along with their response by the deadline below:
1. Letter of interest (including an email
address for notification of additional advertisements).
2. DPW Form
120 or the Standard Federal Form 330.

The Department of Transportation, in
accordance with the provisions of Title VI of the Civil Rights Act of 1964 (78

49, Code of Federal Regulations Part 21) issued pursuant to such Act, hereby

Contractors
are hereby notified of the applicability of Section 11-205.5, HRS, which states
that campaign contributions are prohibited from specified State or county
government contractors during the term of the contract if the contractors are
