NOTICE TO PROVIDERS OF PROFESSIONAL SERVICES
This notice shall remain posted until October 29, 2008
Deadline for Submittals: 3:00 P.M. Hawaii Standard Time: October 29, 2008.
In accordance with §103D-304, HRS, the State of Hawaii, Department of Hawaiian
Home Lands (DHHL), is seeking qualified service providers to conduct land surveys
which include but are not limited to preparation of file plans, legal descriptions
and Land Court documents that are specific to the needs of development projects
as identified by DHHL.
Land Surveying Consultant Services
Professional Services consultant shall provide and delineate useful land survey
information which include but are not limited to preparation of file plans, legal
descriptions and Land Court documents that are specific to the needs of development
projects as identified by DHHL.
Candidates must meet the following minimum specifications:
3 year minimum experience in providing land surveying services to clients.
Any individual or firm interested and capable of providing the professional
service required must submit a letter of interest, together with information
documenting qualifications, experience, work history on similar projects with
at least two (2) professional references, and documentation of the resources
to apply to the project.
Vendor Certificates Required: Applicants must produce proof of compliance as
required by § 103D-310, HRS. The certificates are required as part of the
applicant's submission. The required forms are:
1. Original Tax Clearance Certificate issued by the State of Hawaii Department
of
Taxation (certificate must be less than six (6) months old at the time of contract
execution).
2. Certificate of Compliance (regarding unemployment insurance, worker's compensation,
temporary disability insurance, and prepaid health care) issued by the State
of Hawaii Department of Labor and Industrial Relations; and
3. Certificate of Good Standing issued by the State of Hawaii Department of
Commerce and Consumer Affairs.
In lieu of Items 1, 2, and 3 above, the applicant may also submit an original
consolidated Certificate of Vendor Compliance as issued by the State Procurement
Office via the online system, "Hawaii Compliance Express." Details

Any award resulting from this solicitation shall be subject to the State of
Hawaii General Conditions (Form AG-008). A copy may be obtained from the contact
information below.
Responses shall be mailed or hand delivered; submittals sent via email or facsimile
