The State of Florida, Department of Military Affairs (DMA), Construction and Facility Management Office (CFMO) requests qualifications from State of Florida licensed Architectural /Engineering (A/E) firms to provide complete design for the renovation/alterations of Armory facilities including but not limited to: exterior repairs, upgrades for ADA compliance; upgrades to mechanical, electrical, and plumbing systems; provisions for an emergency generator transfer switch; interior renovations/alterations including ceiling, wall and floor finishes; fencing, parking and site improvements. Also, to provide additional space as warranted and addition/expansion to existing armory building and/or addition of pre-engineered metal building(s).

LOCATIONS: Arcadia and Lake Wales, Florida

PLEASE NOTE: Due to the close proximity of the project locations and the anticipated cost savings to the State of Florida, the Department of Military Affairs will contract with a single entity for both armories listed who shall provide all Architect-Engineer Title I Type A and B and Title II Type C Services. In other words, one firm will be contracted to perform both design projects.

QUALIFICATIONS PACKAGE DUE BY: WEDNESDAY, NOVEMBER 12, 2008 BEFORE 4:30 PM.

ANTICIPATED PROJECT DESCRIPTION: The purpose of this project description is to give the respondents an idea of the anticipated scope required for this project. The final scoping will be defined in coordination with the Department of Military Affairs upon selection.

Services include but are not necessarily limited to the following:

Title I Services:

Type A: Provide field surveys and investigations to obtain data that is essential to the performance of Type B services and that is not available from Owner resources. These surveys and investigations may include topographical surveys; soil borings and other subsurface investigations; mechanical surveys and investigations; determination of utility locations and capacities including phase 3 electrical power availability if applicable; and similar fact-finding investigations and technical studies at the project site.

Type B: To provide an accurate base drawing for the new work, the A/E will confirm existing conditions by performing the necessary on-site reviews and inspections and document accordingly.

Provide production of complete designs (including concepts, preliminary and final plans), drawings and final specifications, basis for designs, design analysis, and preliminary and final cost estimates.

(1) Schematic of Conceptual Design after initial meeting with owner (1st Submittal)

(3) Complete sets of 35% Construction Documents (2nd Submittal)

(3) Complete sets of 70% Construction Documents (3rd Submittal)

(3) Complete sets of 100% Construction Documents (4th Submittal)

(3) Complete sets of Final Construction Documents (Bid Package)

(5) Sealed and signed sets of documents required for building permit application and State Fire Marshal

All documents as required for permitting for FDEP, FDOT and/or City/County Site Plan review Agencies

Architects Preliminary Opinion of Probable Construction Costs (provided at 35% (2nd), 70% (3rd) and final submittals)

Prior to receiving the 70% (3rd) submittal of plans and specifications review, A/E will copy furnish the Department of Military Affairs CFMO Project Manager:

1. A letter sent to the local agency (by the A/E) requesting what type permitting action is required.

2. A list of all pertinent permits.

3. If permits are required other than a building permit, (General Contractor responsibility), the A/E will be responsible to submit for and obtain such permits prior to bid. This is to include local site development review, water management, DOT right of way and/or easement.

This action is to ensure that the General Contractor is able to obtain the building permits at the proper time and there is no delay in starting construction.

Title II Services:

Provide bidding and construction supervision and inspection services, testing, shop drawing review, and management services.

Assist with Mandatory Pre-Bid Meeting and with Bid Opening

Project Schedules (revised periodically as appropriate)

Perform monthly site visits (or site reviews as required) including a written site observation report

Provide timely responses to Request for Information (RFI)

Perform substantial Completion Inspections

Electrical, Mechanical and other required Engineers will be required to provide on-site inspections work progress, at a minimum of three times, or more as required, and provide report documenting respective progress.

Design Features:

Field Documentation

o Geotechnical (Soil) Engineering and /or Testing (Include cost in proposal)

o Land Surveying (Include cost in proposal)

Site Design

o Show proposed new structure(s) and any existing buildings, or structures, all property lines with dimensions, all streets, easements and setbacks. Show all water, sewer, electrical points of connection, proposed service routes and existing utilities on the site. Show all required parking, drainage and grading information (with reference to finish floor and adjacent streets). Indicate drainage inflow and outflow locations and specify areas required to be maintained for drainage purposes. Show north arrow.

o Civil Engineering Design Storm water, Utilities, Grading and Drainage

o Application to FDEP

o Application to City/County (as required)

o Parking for Military Owned Vehicles (MOV)

o Parking for Privately Owned Vehicles (POV)

o Handicap Parking

o Site lighting

Foundation Plan

o Show all foundation and footings. Indicate size, location, thickness and materials. Show embedded anchoring

Floor Plan

o Show all floors. Show all rooms with their use, overall dimensions, and location of all structure elements and openings. Show all doors and windows. Provide door and window schedule or sizes. Indicate all fire assemblies, occupancy separations, fire and draft stops

o Remove and replace carpet

o Remove and replace ceramic tile

Ceiling

o Replace acoustical ceiling tiles and grid (2x2)

Wall Systems

o Exterior

- Pressure clean and seal entire building

o Interior

- Remove or cover exposed conduits and cables

- Refurbish walls

- Paint entire interior

- Replace/refurbish doors

Plumbing

o Show all fixtures, piping, slopes, materials and sizes. Show point of connection to utilities

o Plumbing facilities to conform with code

o (1) Handicap toilet will be provided for male and female latrines each

o (9 Minimum) Shower heads with reasonable dressing areas will be provided for male (minimum 5) and females (minimum 4) latrines each

o Plumbing facilities to be replaced or upgraded

o Water Coolers will be replaced and conform to ADA

Mechanical

o Show the entire mechanical system. Include all units, their sizes, mounting details, all ductwork and duct sizes. Indicate all fire dampers where required. Provide equipment schedules.

o Mechanical facilities to conform with code

o Unless otherwise directed, all HVAC to be central air with heat pumps

o No A/C units on roof unless otherwise directed

o Include A/C zones based on functional use

o Space may be required for new mechanical rooms.

Electrical

o Show all electrical fixtures, (interior, exterior and site) wire sizes and circuiting, grounding, panel schedules, single line diagrams, load calculations, and fixture schedules. Show point(s) of connections.

o Electrical facilities to conform with code

o Non-compliant electrical systems including wiring, conduits and electrical panels to be replaced or upgraded

o Lighting to be replaced with energy efficient lighting

o Include manual emergency generator transfer switch complete with working connection box; include testing

o Telecommunications including telephone and data to be complete working system

Fire Detection and Suppression

o Tested and working system certified and approved by State Fire Marshal

Recruiter Office

o Wainscot, chair railing and crown molding

Pre-Engineered Storage Building

o Provide complete design detail including foundation, structural and electrical

o Provide all documents for permitting

o Storage building will be used to replace conexes currently being used to store equipment and supplies Determine size accordingly.

o Storage compartments must be secure Walls of compartments, whether chain-length fence or other, must be from floor to ceiling

o Provide necessary water management facilities (drain spouts and grading) to prevent water from flowing into the building.

PUBLIC ENTITY CRIME INFORMATION STATEMENT: A person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for the construction or repair of a public building or public work, may not submit bids on leases of real property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017, for Category Two for a period of 36 months from the date of being placed on the convicted vendor list.

Requests for private meetings by individual firms will not be granted. No individual verbal communication shall take place between any applicants and the Owners except as provided at the Pre-Submittal Meeting. Answers will be distributed to only the firms, who provide a valid e-mail address on the sign-in sheet. Requests for any additional information, clarifications or technical questions must be requested in writing. Prior to entering into a contract with the State of Florida, Department of Military Affairs, the selected firm must be registered with the Florida Department of Management Services (DMS) MyFloridaMarketPlace Vendor Registration System. Registration must be completed at the myflorida.com MyFloridaMarketPlace http://dms.myflorida.com/business_operations/state_purchasing/myflorida_marketplace. INABILITY TO POST ELECTRONICALLY: If the Department is unable to post an Intent to Award or Intent to Negotiate (due to technical difficulties) as defined above, the Department will notify all firms via the Point of Contact and electronic address provided to the Department. Notice will be posted as defined above once the technical difficulties have been rectified; however, the official posting time will be that time at which the last firm was electronically notified.

The STATE OF FLORIDA requires all Vendors to implement a drug free workplace program as defined in 287.087, Florida Statutes.

MINORITY PROGRAM: Minority Business Enterprises (MBE) are encouraged to participate in this Request for Qualifications. Utilization of MBE participation is highly encouraged from all participants. MBE's must be certified by the Office of Supplier Diversity.

The Department reserves the right to accept or reject any or all proposals received and reserves the right to make an award with or without further discussion of the proposals submitted or accept minor informalities or irregularities in the best interest of the State of Florida, which are considered a matter of form and not substance, and the correction or waiver of which is not prejudicial to other Proposers. Minor irregularities are defined as those that will not have an adverse effect on the Department's interest and will not affect the price of the proposal by giving a Proposer an advantage or benefit not enjoyed by all other proposers. It is understood the proposal will become a part of the Department € s file, without obligation to the Department. Proposals may be rejected, in accordance with Agency standard € s and found to be irregular (not in conformance with the requirements and instructions contained herein). A proposal may also be found to be irregular or non-responsive by reasons that include, but are not limited to failure to utilize or complete in their entirety prescribed forms, conditional proposals, and improper, missing and/or undated signatures. All proposals deemed non-responsive will be disqualified.

CONTRACT AWARD: The short list for interviews or the order of negotiations of the selected firms will be posted on the My Florida website http://vbs.dms.state.fl.us/vbs/main_menu. Failure to file a protest within the time prescribed in section 120.57(3), Florida Statutes, shall constitute a waiver of proceedings under Chapter 120, Florida Statutes. If no protest is filed, a contract will be negotiated with the firm which the agency considers the most qualified for the intended project. If an agreement for compensation which the agency determines is fair, competitive and reasonable cannot be reached and the agency is unable to negotiate a satisfactory contract, negotiations will be formally ceased and negotiations shall be undertaken with the firm the agency has determined is the second most qualified after Departmental review of the submitted Professional Qualifications Supplement (PQS).

Selection of applicants for consideration will be made on the basis of professional personnel; professional services qualifications including related construction/design experience and ability, location, financial capability, scheduling/cost control, past performance, and recent, current, and projected workloads of the firm. The Qualifications packages will be reviewed and shortlisted to three firms. Reference checks will be done on three shortlisted firms. Telephone or in-person interviews or presentations may be required to determine the best qualified firm for this project.

Ranking of firms for oral presentations will be done by the information provided on the Professional Qualifications Supplement (PQS) submitted with your package (50% of score). The balance (50% of score) of scoring will be based upon DMA technical representative € s assessment on your ability on these projects. The PQS form DBC 5112 has been attached to this advertisement for download from the Vendor Bid System on the MyFlorida.com website or by contacting the Contracting Branch below.

SUBMITTAL DUE DATE: Wednesday, November 12, 2008 before 4:30 p.m. Submittals received after this date will not be considered. However, submittals may be sent to the CFMO any time prior to the final due date.

1. A Letter of Interest detailing the project locations firm is submitting for, the firm's qualifications, related experience and the firm's abilities to do the work, and to meet the above referenced selection criteria.

3. Firms must be properly registered at the time of application to practice their profession in the State of Florida.

Question #6 in PQS form: A list of at least 4 previous or current projects which

exhibit your firm's experience as related to his project. You must have completed at least 2 projects of the same size scope and complexity within the previous 3 years to be considered qualified. Provide contact information of the Owners and contractors. Describe work performed on projects in detail.

3. For Corporations only: If the firm offering services is a corporation, it must be properly registered with the Department of State to practice their profession in Florida and must provide one copy of the firm € s current Florida Corporate Registration.

Facsimile (fax) or e-mail submittals are not acceptable and will not be considered. All instructions must be complied with and requested data must be included in order for your firm to be considered for future projects. All information received will be maintained with the department and will not be returned.

The State of Florida's performance and obligation to pay under any contract is contingent upon availability of funding and an annual appropriation by the Legislature.

Registration in the MyFloridaMarketPlace system on myflorida.com is required in order to receive an award from a state agency.
