Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented; a written solicitation will not be issued.    The RFQ number is CH-MS08-006.    The NAICS code for this action is 621999.    All offers received in response to this solicitation will be considered.    Email responses to margaret.shirley@ihs.gov no later than 1500 hours February 29, 2008.
Item #1: 500 Regular hours @ __________; for the provision of Community Nutritionist service.
The performance period is March 01, 2008 through September 30, 2008.    The personal services contract for Internal Medicine/Family Practice is to be provided at DHHS-IHS-Chinle Comprehensive Health Care Facility, PO Drawer PH, Off Highway 191, Chinle, Arizona    86503, in accordance with attached scope of work.    Please be advised that all contractors doing business with the Government must be registered with CCR (www.ccr.gov).    (FAR) The provision 52.212-1, Instructions to Offeror-Commercial Items, as stated in this provision, the Government intends to evaluate offers and award a contract without discussion with offerors.    Therefore, the offerors? initial offer should contain the offeror?s best terms from a cost or price standpoint; however, the Government reserves the right to conduct discussions if later it is determined by the contracting officer to be necessary.    The following provisions apply: 52.212-1 Instructions to Offerors; 52.212-4 Contract terms and conditions; 52.224-1 Commercial Items; 52.224-2 Privacy Act Notification; Privacy Act 52.224-2; Required CCR 52.204-7; Availability of Funds 52.232-18; Continuity of Services 52.237-3; Termination (Personal Services) 52.249-12; Payment by Electronic Funds Transfer Central Contractor Registration 52.232-33.    Referenced provisions and clauses may be accessed electronically at these addresses: http://www.arnet.gov/far; http://farsite.hill.af.mil; http://www.dtic.mil/dfar.    Overtime is not authorized.    The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered.    General information: Only the contracting officer has the authority to approve changes to this contract.
Community Nutritionist Scope of Work (11/07)
The purpose of the Public Health Nutritionist position is to help plan, coordinate, implement, and evaluate community-based nutrition care, education and promotion of services.    Under the guidance of the Supervisory Public Health Nutritionist or designee, the incumbent helps coordinate the provision of services and the development and implementation of innovative programs through collaboration with Dietary Services, Health Promotion, Public Health Nursing, Diabetes Program, and other departments and programs in order to meet the needs of the SU and the community for which it serves.
The contractor:
?      Reviews, evaluates, and arranges to acquire suitable educational materials on basic and specialized nutrition topics, as identified by community assessment, for use by professional and paraprofessional staff providing services to clients at the Wellness Center and in the community.
?      Provides consultation and technical assistance to the Clinical Diabetes Program, Health Promotion Department, and other professional staff to help coordinate service unit wellness programs and other diabetes prevention programs.
?      May act as technical consultant to local non-IHS programs and community groups on nutrition-related issues.
?      May serve as liaison, preceptor or advocate for public health nutrition in the Hospital or related tribal programs on interagency, tribal, regional and professional committees, particularly in addressing the nutrition needs of the community and the recruitment and retention of nutrition students and interns.
?      Helps review and assess program progress and operations to identify and make recommendations for improving the quality, access, and appropriateness of community nutrition services and programs.
?      Participates in developing and implementing Department Performance Improvement Plan.
?      Reports and summarizes progress and activities at regular intervals; prepares periodic and special reports or projects.   
?      Coordinates with Dietary Services, Clinical Diabetes Program, Health Promotion and other departments so that comprehensive, effective nutrition services and programs are provided through out the service unit through active committee participation.
?      Coordinates and collaborates with a variety of organizations to improve the nutritional health of the population, such as chapters, schools, businesses and other community groups.
?      Works with Navajo Nation programs providing community nutrition-related services in Chinle Service Unit, such as the Special Diabetes Program, Community Health Representatives, WIC (Women, Infants, & Children), and New Dawn programs.   
?      Can provide medical nutrition therapy to individuals and nutrition education to groups in non-hospital or health center based clinical setting or community based setting.    Examples are schools, institutional and faith based settings, mobile care unit, homes, chapters, or community centers.
