Addendum # 1 - Removes the pass/fail criteria, 5.1.3, that the Proposer shall be legally
authorized to transact business in the State of Oregon in order to submit a Proposal in
response to this RFP. By submitting a Proposal, Proposer certifies that if awarded the
contract, Proposer will be authorized to do business in the State or Oregon at the time of
award.
Opportunity Summary
The State of Oregon, acting by and through its Oregon Youth Authority (OYA or Agency),
requests written proposals for ongoing Organizational Assessments of the Agency. The goal
of these services is to assess staff climate, culture and management practices in
facilities and offices through surveys to staff. The results of the survey will drive an
agency improvement plan and establish a baseline for ongoing quality improvement.

The mission of the Oregon Youth Authority (OYA) is to protect the public and reduce crime
by holding youth offenders accountable and providing opportunities for reformation in safe
environments. The majority of the 1150 employees work with youth in eleven Youth
Correctional Facilities across the state as well as Probation and Parole Field Offices
covering Oregon's 36 counties. The centralized leadership and administrative staff are in
Salem, Oregon.
