The National Hansen's Disease Programs (NHDP) has a requirement for an activities coordinator for institutionalized geriatric patients as well as to short-term patients in active medical and surgical care. Many patients at the NHDP are physically handicapped and/or emotionally disadvantaged and represent a wide variation in socio-economic, ethnic, cultural, educational, and vocational backgrounds. Patients often experience a relatively long stay away from family and support systems in a hospital setting many miles from their home. Specifically, this contractor will provide services to the National Hansen's Disease program in the following areas: (1) Plan, implement, and coordinate a comprehensive program of daily activities for a multi-cultural, mostly geriatric population with varied levels of ability/disability in individual and group settings, both at the NHDP and on community outings. (2) Implement JCAHO guidelines, document general program content, make individual assessment and activity plans for each resident addressing long and/or short-term goals, and periodically re-evaluate the efficiency and effectiveness of each plan modifying goals as necessary. Document activity plan and participation in plan. (3) Serve on a multidisciplinary care team to provide the most appropriate total care plan for each resident. (4) Document the funds used throughout the year. (5) Coordinate Activities Program volunteers and Patient Employees to provide parties, group activities and community outings and staff the activities room. (6) Maintain liaison with community resources including other groups and agencies to best utilize those resources for NHDP residents. (7) Coordinate and execute individual, small and large group activities, including but not limited to bingo games, parties, movies, meals, shopping trips, dining out, arts and crafts, reading materials and dominoes. Services will be provided at the National Hansen's Disease Programs location at 1770 Physicians Park Drive, Baton Rouge LA. Qualified contractors must satisfy the following minimum requirements. (1) Successful completion of a full 4-year course of study in an accredited college or university leading to a bachelor's degree of study. The degree must be in recreational therapy or in a major in an appropriate subject matter field with a therapeutic emphasis or concentration. (2) Communicate effectively in both English and Spanish. (3) Experience with JCAHO guidelines. (4) Experience with Patient Assessment and documentation of activities plan. Services required 30 hours per week hours per week for one year from date of award, plus an option through September 30, 2009. This is a 100% small business set-aside, restricted to small businesses eligible to receive award. The RFP will be posted at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, BPHC/National Hansen's Disease Programs, Posted Dates on or about July 18, 2008. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The NAICS code for this requirement is 621340 Offices of Physical, Occupational and Speech Therapists, and Audiologists. Solicitation will be conducted under Federal Acquisition Regulation (FAR) Part 12 and 13. The closing date for receipt of proposals will be approximately thirty (30) days after issuance/posting of the RFP.
