DESCRIPTION: This is a combined synopsis/solicitation for services. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-249-09-RQ-0027, is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 325120. This procurement is full and open competition. Quotations are due on or before November 14, 2008 by 12:00 PM Central Standard Time to be considered responsive. Item Description: Dry Ice Delivery. The dry ice will be sliced 1" thick, 50 pounds per block, 4 blocks per delivery. Dry ice will b e delivered every Tuesday and Friday. If delivery date falls on a holiday, please deliver on the following business day. For Friday's please deliver one (1) day ahead (Thursday). The hours of delivery is between 8:00am and 4:30pm. National Holidays: The ten (10) holidays observed by the federal government are: New year's day, Martin Luther King's Birthday, president's day, memorial day, independence day, labor day, Columbus Day, veteran's day, thanksgiving, Christmas day. Base period will be 12-1-08 to 9-30-09. Option to extend 4 years. Option year period will be October 1 to September 30. Delivery shall be FOB DESTINATION to VA Medical Center, 1030 Jefferson Ave Memphis, TN 38104. Offers are due by 12:00 PM CST on November 14, 2008. The assigned Purchasing Agent is Hollie Dickey.
